Happy water overdose: A trending but fatal street drug

“Happy water” has been gaining popularity among party goers. The effect of consuming this drug can be fatal. Two patients of “Happy water” overdose were admitted for reduced level of consciousness after enjoying a farewell party at nightclub. Urine for toxicology screening was found to be positive for methylenedioxymethamphetamine (MDMA), methamphetamine, diazepam and tramadol.
“Happy water” is a synthetic drug cocktail emerging across Southeast Asia, particularly prevalent in nightlife scenes. The cocktail is a poly drug mixture packed in sachets or disguised as instant coffee or beverage mix. Health experts and law enforcement claim strongly that these mixtures are not only unpredictable in effect but also pose acute risks of overdose, collapse and death2.
Diagnosis of “Happy water” overdose can be challenging due to its varied clinical presentation because of varying combinations and concentrations of psychoactive substances; sedative, stimulant and hallucinogenic elements. Proper history taking, recognition of toxi-dromes and presumptive drug screening can make a correct diagnosis3.
Management of “Happy water” overdose requires urgent multidisciplinary medical intervention due to its unpredictable composition leading to life threatening complications.
